DC3 Permission to Participate Form (PDF)

This form is required for all persons under the age of 18, reguardless of country of residence or citizenship, to participate in the DC3 Challenge as per the Challenge rules.
Once completed by a parent/guardian, this form can either be faxed or emailed to us as per our contact information.
